My working kit for the trip I just returned from included three lenses (DA21, FA35, FA77) and the DS body.
I used a Tamrac SuperLight 5 case to work out of. I ripped out all the standard dividers and fitted two at the bottom of the bag to provide 'drop in' compartments for three lenses. On top of that I folded up a Domke Wrap to provide a soft "cover" for the lens compartments that the camera can sit on with a lens fitted. In addition, I slipped a climbing carabiner onto the shoulder strap to work in conjunction with the wrist strap I always have on the camera. Like Bob, I remove the front caps when I buy a lens and fit a screw- in metal lens hood that sits permanently on the lens unless I put the lens into storage for some reason. Lenses drop into their 'slots' in the bottom of the bag hood down protecting the front element. If I'm working quickly, the rear cap is left off to minimize the possibility of losing them or scratching the rear element (the folded Domke Wrap will protect the back of the lens if you keep it clean). Normally, I work a little slower and replace the rear caps as I switch lenses for maximum protection. I use three slots for carrying two lenses and one on the camera because that way you can always drop a lens into an available slot. I can also carry an additional lens that way. The carabiner comes in handy if you need to juggle two lenses and a rear cap, want a third hand to hold the camera body. I just hang the body on the carabiner with the wrist strap. Another thing I'm going to do is buy six rear caps and glue them together into pair. That will make it easier when doing a quick change to transfer the cap from one lens to the next ... just connect the lens you just took off to the one you just pulled out, then twist the one going to the camera off and drop the other back into the bag.
