A Rohloff hub is what Lahar uses :)

They mount their gearbox in the frame to reduce unsprung weight and  
to help keep the bike's weight centralised which improves handling.   
The reason they used a gearbox in the first place is because it  
removes a few compromises from the rear suspension design.

I've also seen some funky old motorcycles that have an in-hub  
engine.  Not to mention those crazy contraptions where the rider sits  
inside the wheel.
