I've been using Vuescan for a very long time.

The two that make a difference to me are the ability to output RAW  
files and the unlimited upgrades support. Ed updates Vuescan  
frequently with bug repairs, improvements and to adopt new scanners  
so the unlimited upgrades is a valuable plus. Also, the ability to  
put out its own RAW format files means that you can physically scan  
media once and reprocess it with great flexibility many many times to  
make the best scan for editing in Photoshop (and now Lightroom!).
