When you enable Automatic XMP updating, Lightroom will run through  
and touch every file in the library, updating the metadata with any  
and all changes. After that, LR will constantly synchronize metadata  
from its database to the actual files. Whether you're adding  
keywords, cropping, doing tonal adjustments, etc, all of these  
changes are stored in the metadata as you go along so it gets into  
quite a bit of library to file back and forth.

If you have the Automatic XMP updating turned off and want to access  
those same files in place, say with Bridge and ACR, go to the folder  
you want to edit in, select Library/Grid mode, select all the files,  
and choose the "Metadata->XMP->Export XMP Metadata To Files" command.  
Lightroom will force all the metadata to be updated. You can then  
quit Lightroom and open a view on that folder with Bridge. Both  
Bridge and Camera Raw 2.7 will read and interpret the metadata  
properly. If you make changes on files in that folder with Bridge and  
Camera Raw, when you restart Lightroom and go to that folder, select  
all the files and use the complementary 'Import XMP Metadata From  
Files' command (same submenu). That will refresh LR's database with  
the latest settings.

You should also do this if you have the same folders in two different  
Lightroom libraries.

For other applications beyond Bridge/Camera Raw/Photoshop, the same  
thing will apply. You can always Export a DNG too: Lightroom will  
embed all the current metadata settings at the time of export unless  
you turn off metadata embedding.

I do combinations of the above as I occasionally use Bridge to run  
some scripted outputting, but generally have my files organized in  
the file system such that they are in separate Lightroom libraries so  
I don't get into the "which set of Lightroom edits is the correct,  
current one".

Yes, unless you have forced a metadata export as above.
