You're right it happened exactly once to me, that I can recall for sure, on 
30 or so images that were all of the same subject and lighting.

Regarding .jpeg and RAW comparisons.  Apples and oranges really.

I know you're actually referring to percieved noise in an image, but testing 
for noise by shooting inside the lens cap is actually a method not unlike 
the dark frame subtraction used by the noise reduction functionality, and is 
ubiquitous in astrophotography. That's where it will *really* show up.
