Looks like a hard drive failure happening in slow motion. However before replacing the hard drive, replace the cables. They can go bad at any time. (It's the easiest and cheapest thing to try first). Do it soon as bad cables can screw up your OS installation, damage data and make you wish the HD had failed. If this doesn't fix the problem assume the HD is dying and get your data off it ASAP. I had a drive partitioned into three virtual drives, with a live image of the OS on a second smaller drive. The original drive failed and completely screwed the system partition. Luckily the non OS partitions remained readable. I was able to transfer the OS image from the smaller drive to a new drive and recover all the data before the original drive went completely toes up.
