You're not required to send in the warranty card. Just send your lens to Pentax 
Colorado along with a description of the problem and the receipt. I would send 
both the "thank you for your order" message and the credit card receipt if you 
have both, but I think either would suffice. Check the Pentax USA for specific 
return instructions.
