I'd be slightly hesitant to include all of that. Looking over a gallery of images taken with various lenses and bodies, all containing my address would be a bit much like a big flashing sign saying "Look at all the camera gear I own. And here's where I keep it all." But certainly sufficient info to identify it as yours is a good idea.
