Then I suggest you look for some PDF templates like the ones you find here:
http://www.calendarsquick.com/printables/premium.html

Import PDF to PS, enlarge the canvas to make room for your image, and
downscale to paper size afterwards.

He has an example of a portrait-style calendar with image above the date fields.
If you choose "image orientation: portrait" in the options dialogue,
you will get what you request.
Alternatively, I suppose you can increase the canvas size as for PDF
templates, and then just move the picture layer onto the vacant area.
Haven't tried that myself yet though.
