Think I've seen 'em in Vista, but never one so bad that I'd had to reload Windows. The most I've had to do is cycle power.
Most of the time it doesn't even require that. It'll come back up by itself after a few seconds with a message that the "video driver stopped working momentarily."
Seems like any process that uses a lot of memory can trigger it, especially embedded video.
