I had a similar thing happens with a 1.4L T/C several years back. In that case, it stuck open and blew out images. I manually activated the aperture blades and confirmed the problem and sent it to Pentax repair. The first time I checked it after getting it back, it stuck. I tried and tried to get it to stick again, but was unable to. I used it on an A 300mm f/2.8 for some years after that with no problem. However, I don't think I ever used it after that without checking and hoping.
