It depends on how critical/neurotic you are. LCD monitors are pretty
stable and if you're not sending files out for extremely critical fine
art printing I'd say once a month is fine.

Nope. If you're using color managed software like Photoshop the ICC
profile for ProPhoto RGB will take care of the rendering.

Calibrate and profile your monitor for 2.2 gamma and 6500K white
point.
