It's more a sad reflection on human nature. Somebody will always try to cut corners, skimp on inspections, etc. It doesn't matter if it's a nuclear power plant, a deep-sea oil well, a copper mine, a gas pipeline, ... There's no accountability, and no personal risk - It's only somebody elses life on the line.
