note the date format "2011/2011-04-17" -- the slash here is telling you it's going to create both a 2011 folder and a 2011-04-17 folder _within_ the destination folder (which is 2011); this is why you are getting two levels of 2011 folders
in the date format pop-up, see if there's an option which reads simply "2011-04-17"
