I'd be tempted to make a block with one face 45 degrees to another, and mount a 
plate that would fit in the hot shoe. Slide the block into the hot shoe, and 
set a level on the top. 

One of the round ones with a bubble in the middle might be best, so the camera 
is both level and and angled down at 45 degrees.
