I can see some blotchiness in the web image - maybe it is more obvious in the the full res version.

Here's something you can try in LR that works well as a "skin smoother". In the develop module, go to the adjustment brush. In the effect section, set the exposure to -4, the clarity to -100 and the sharpness to =100. (Setting the exposure to -4 just allows you to see where the brush has gone so don't be concerned about how it reduces the brush stroke to black)

In the brush section adjust the size and feathering as necessary and set the flow to about 75 or so.

Brush over the area you want to smooth, then change the exposure back to 0 to see if the effect is what you want. If it's not to your liking just go back in the history to where you started and try again. You can adjust how strong the effect will be by increasing or decreasing the flow setting.

Would you address the blotchy?  Or leave it?
What would your go-to method be?
