Had to do it with a medium format camera and B&W film (which we also had
to process & print). Digital "polaroids" not permitted (automatic F for
cheating).

That is, without a doubt, the best book on lighting I have ever read. It's not just a cookbook, but it clearly explains the principles behind the techniques and how, and why, to apply them.
