That's what TimeWarner does with RoadRunner, although they claim they don't.

Can't tell you about Canada, in the US it really depends on your ISP
and local PUC. I suspect that if you use too much bandwidth during a
monthly period that AT&T squeezes the intertubes to slow usage until
the end of that period.
