http://www.majorleagueelectronics.com/, they both do short run custom
PTH and SMT connectors. (Major League was set up by an ex samtec employee
with a baseball fixation)

If you are after high speed (but only standard parts) then I think FCI
http://fciconnect.com do some hot plug application parts.
And if you can find your way through Tyco/AMPs online catalogue AND they
actually have ever made the part, then they can probably help

I assume what you are looking for is one pin that is longer than the rest,
(or a recessed socket pin) for hotplug applications.
