To answer your first question, yes it's possible that the ways of input
are switched between imperial and metric units. However this wont be a
problem if you unswitch them by using key appropriate.

If components must be mounted on the PCB with screws, then use pads.
Pads have holes, screws require holes, therefore screws may require
pads. Pads are the best option to use, they can have zero annular ring,
be marked as unplated and untented.

If you only need holes then just set the pad size to 0. Have the hole
size as necessary.

And finally... dear god, ring up your Protel/Altium reseller and ask for
a tutorial on using the package. I really have never met an engineer of
any kind that appears as incompetent as you... and that includes civil
engineers :P
