I think you'll be looking at around 8 to 16mm.
The exact distance depends on the conditions expected to be experienced 
by the board, and what the specific peak voltages are.

You'll certainly want to check with your board house to make sure that 
they have solder mask that will withstand this voltage, most are good 
for 500V/mm or so, but you'd likely want a bit more than this to account 
for deterioration over time.  I'm sure others will have some more 
accurate details.

You should check with your local certification company, especially if 
you're going to market in several countries.
