Syarat nya yg saya tau di perla medan merdeka.
1.Buku pelaut asli dan foto copy
2.Sertifikat SCTW 78 (jika dulu dari Amk-pt) copy dan asli
3.Sertifikat Pengukuhan asli
4.Sertifikat SCTW 95 Asli dan fotocopy
5.Pasfoto 2 lembar ukuran 3X4 warna.
