and because I am interested in effects of inequality on numerous indicators

but I cannot locate it successfully on Amazon or Abebooks

and a link to this discussion article clearly by the same author apparently
after the Bush election victory.

Regarding "high value added:" This phrase is usually coupled to an ambition
to "move up" the value chain.  The higher value added is associated with
things like design and branding, remote from where most labour value is
added.  Consequently this phrase actually means "high value captured" from a
Marxian perspective.
