Rocky Flats was a for-profit plutonium processing facility that lost track of some pounds of plutonium.  Maybe it it outside on the ground, maybe in the piping.  I went to a demo there once, probably 1979.  The place was shut down and the contractor ( ?? can't remember who) walked away without much or any penalty for failure to keep track of the nuclear material.
    Now they want to turn it into a park.  The investigation into the finances and the missing nuclear was widely believed to be a whitewash.  One of the investigators has just blown the whistle on it again.  Maybe just published a book.  I'm not drinking enough to have a clear memory.
