This patches Test::More, Test::Simple, Test::Tutorial and Test::Builder to refer to
'names' as 'comments' both in the docs and in variable names.
The only place I did not change was the details in the $TestResults array in Test::Builder.
There is a hash key called 'name'. I assumed that changing it to 'comment' would break
things :)
