This is not the first time this has happened. I'm pretty sure it is broken and not old (isn't PERL5_CPANPLUS_IS_RUNNING pretty recent?) It would be nice in general if FAIL reports could be crossed-off or deleted by humans. Some feedback to the tester would also be good (how do I contact a tester?) Some way to link a failure report to a known/new bug in RT would also be pretty nice.
