Dest buffer needs to be bigger for utf16_to_utf8()
These undocumented functions require the destination buffer to have the
worst case size. However that size (previously listed as 3/2 * input)
is wrong for EBCDIC. Correct the comments, and the single use of these
in core.
These functions do not have a way to avoid overflowing, which strikes me
as wrong.

Change some I32 into SSize_t or bool
This converts some I32 I stumbled across to either SSize_t when length
was meant, or to bool when true/false was meant.

POSIX.xs: Fix to work better multi-threaded
This defines POSIX_EXT at the beginning of the file which automatically
causes any reentrant versions of the functions called here to be used.
It also adds critical sections around several function calls that need
them in multi-threaded applications. These functions are all
locale-related, and while always broken, it was forbidden to use locales
in multi-threaded contexts, so the issue never came up. This is in
preparation for making locales multi-thread capable.

POSIX.xs: Move constant tests out of loop
Whether or not a locale is in UTF-8 doesn't change in this loop;
calculate it once, prior to the loop
