perlintern: Combine refcounted_he_fetch entries
This removes redundancies in the pod and makes it easier for the reader
to choose which is best for their needs.

perlintern: Combine hv_refcounted_new entries
This enables the reader to see all the similar possibilities at a
glance, to compare and contrast them, and hence to choose the best form
for their needs.
