perlxs.pod: mention blank lines after keywords
Mention in a couple of places that there has to be a blank line between
a file-scoped keyword and a CPP directive, e.g.
PROTOTYPES: ENABLE
#ifdef FOO
and mention that MODULE used to be an exception but is now like other
file-scoped keywords.

perlxs.pod: mention that TYPEMAP doesn't do POD
Mention that a TYPEMAP: heredoc block doesn't recognise POD etc.
Also update the code comments for fetch_para() to reflect the same.

perlxs.pod: clarify "duplicate XSUB" conditions
Since 3.61, the circumstances where a "duplicate XSUB" warning are
emitted have been relaxed. Update the docs to reflect this.
