Erin, I would say that this looks like moisture damage. It likely includes mold, but could also be a deposit of whatever was in the water that touched the book, or could be migrated colorant from something else that got wet, or it could be efflorescence from the cover material getting damp. Consult a book conservator about the extent of the damage--it might be something that could be cleaned off the surface, or the cover might need to be replaced.
