I do not do any thing special in configure stage for Fortran. So if you have a Fortran code and call petsc from with in, it should work. Just use same compilers for bulding petsc and your code. There a number of Fortran based examples in petsc which i very often refer to.
Most of my code is Fortran 2003 compliant and to your second question, i don't clearly understand it but i think yes.
