A year ago I was told by a member of the PB development team that there
could be only *two* reasons for the "Creation of Executable Failed" message
after a successful rebuild: 
 
1. Invalid Icon File (or wrong path)
2. Unable to write to a directory specified in the project painter.

Not sure if that still holds true today, but I suspect that in your case PB
may not recognize the icon as a valid .ico file.   Try to open it in the
Watcom Image Editor.

At one time in 5.0 there was in issue causing the same message if the
application icon was not path-qualified, and the icon was not in the
current directory, but in the path. The workaround was to add a
fully-qualified path to the icon, exit PowerBuilder and restart.
