[ADMIN] Index pg_attribute_relid_attnam_index is not a btree

What would be causing the following error on a system that has been running
fine for months.

>       ERROR:  Index pg_attribute_relid_attnam_index is not a btree
>       createdb: database creation failed on vioxxdocs.
