Return NULL instead of 0/0 in pg_last_xlog_receive_location() and
pg_last_xlog_replay_location(). Per Robert Haas's suggestion, after
Itagaki Takahiro pointed out an issue in the docs. Also, some wording
changes in the docs by me.
