Fix ALTER SUBSCRIPTION grammar ambiguity There was a grammar ambiguity between SET PUBLICATION name REFRESH and SET PUBLICATION SKIP REFRESH, because SKIP is not a reserved word. To resolve that, fold the refresh choice into the WITH options. Refreshing is the default now.
