Then we may do the second part as a loop and hope that eventually we hit
the right point with either INSERT or UPDATE:

 UPDATE
 WHILE NOT FOUND THEN 
   INSERT
   IF DUPLICATE KEY THEN
     UPDATE
   END IF
 END WHILE
