[PATCHES] ignore_killed_tuples is always true

I found IndexScanDesc->ignore_killed_tuples is always true.
Is this still needed?

Also, I cannot understand why gistgetmulti calls gistnext with
ignore_killed_tuples = false. We can always ignore LP_DELETEed tuples, right?
