I recently downloaded the 0.43 source and when going to make pgserver, it would come up with an error about not finding "SDL/SDL.h", but to cut back a very long story, i ended up figuring that i did have that header, and there were 2 ways to fix this, either in the source change the include to: #include <SDL.h>, or leave that as is, and in sdl-config for the -cflags option, add another includes directory ("-I/usr/X11R6/include", as opposed to just "-I/usr/X11R6/include/SDL" which was the only one there before). After this, make ran perfectly. So i'm not sure if this is just me who has this problem, figuring noone else has complained, not sure why it would just be me, but this is just a simple fix for anyone who comes across it.
