That works here using both my rather old Pharo development image and pharo0.1-10259dev09.03.3.zip. Any chance you could run it under gdb with a breakpoint on error to get the C backtrace? Looking at the source to the primitive it's very simple, I'm not sure how it could fail so long as the UUID generated is 16 bytes long.
