Here's the answer I received from Mr. Dethlefson:

"I'm afraid that the information you want is at the Edison Historic
Site in New Jersey and the Site is closed for renovation. What you
want will require a trip to NJ and much digging in the archives when
the Site re-opens.

I had asked him additionally about the number of takes Edison might have 
required for BA production, so it sounds like maybe there's a chance there 
might be more than one take available on BA 5011 as well.  Again, does 
anyone know what Edison's policy was regarding the BA's?  And do BA's have 
take numbers hidden anywhere on them?
