The version of PHP that this bug was reported in is too old. Please
try to reproduce this bug in the latest version of PHP (available
from http://www.php.net/downloads.php

If you are still able to reproduce the bug with one of the latest
versions of PHP, please change the PHP version on this bug report
to the version you tested and change the status back to "Open".

I should've mentioned that the script contains just 

<?php phpinfo() ?>

this is actually 4.0.7RC1
Server: Apache/1.3.20 (Win32) PHP/4.0.7RC1
php_oci8.dll linked against the 8.1.7 client libs

hitting the server with 
$ ./ab -n 5000 -c 3 -k http://212.24.143.100/other/info.php

produces more than a handful of these:

the errors go away with the extension disabled.
