Bug description:  Bad field length in header (type D and L)

Description:

When create or read or write qBase file, in header write field length for
D and L type = 0

Reproduce code:

$def = array(
  array("date","D"),
  array("log", "L")
);
$file_name = 'c:/test.dbf';
dbase_create($file_name,$def);  
//length D and L in header is 0 IS ERROR
?>
