This bug has been fixed in CVS.

In case this was a PHP problem, snapshots of the sources are packaged
every three hours; this change will be in the next snapshot. You can
grab the snapshot at http://snaps.php.net/.
 
In case this was a documentation problem, the fix will show up soon at
http://www.php.net/manual/.

In case this was a PHP.net website problem, the change will show
up on the PHP.net site and on the mirror sites in short time.

I'm not sure if this relates only to IIS -

I upgraded to 4.2.1 and fileupload didn't work at all so then I used
4.2.3. It's better but still file upload seems buggy. The simplest test
I could create is the script below, which generates an endless number
of php$HEXNUMBERINC.tmp in the upload temp directory. Each file is
about 4.999 KB in size. The only way I could find to stop it is to stop
IIS.
