After some experimentation I figured it out. do-download.inc calls flush() on line 31. This causes the 'original' status to be sent instead of the one specified by the call to header(). If 4096 bytes or more are written before the flush the correct status is sent. I guess the codepath that handles an expicit flush manages to loose the new status code somewhere. The client always receive 404.
