Bug description: DOMEntityReference->__construct crashed when called explicitly
Description:

DOM XML classes contain __construct methods that behave in a quite
unexpected way. You can call the constructor explicitly ending up with a
broken object (e.g. "Couldn't fetch DOMAttr. Node no longer exists"
whenever you access a method/property of the object).
Nevertheless, the constructor of DOMEntityReference, which is the subject
of this report, is broken completely.
