The patch was integrated into 4.0.3 (I think), and "solved" our problem. 

In 4.0.4 part of the file-upload code appear to be rewritten, and a
simular (but not the same) problem has arisen. Basicly the mime-header
parser is crude and ugly code that only handles a subset of the legal
header-set.
