I'm experiencing the same problem. OS: Windows XP Apache 1.3.27 for Windows MySQL 3.23.53-win PHP 4.2.3 for Windows (php-4.2.3-Win32.zip)
After I successfully installed Apache and MySQL, I tried to install PHP as module. Following the instructions of install.txt I got this result: Cannot load [path]/psp/sapi/php4apache.dll into server: module not found. The problem lies somewhere within the line 'LoadModule php4_module c:/[path]' - but if I look into the .dll the module is there.. Regards, Jan 'Mac' Maska P.S. No 'RTFMs', please.. this error was generated using your installation notes.
