One other possible scenario:

You *think* you've turned off all those Magic Quote things, but you
are using PHP as a Module, and forgot to re-start Apache, so you
really didn't turn them off.

Confirm 100% sure with <?php phpinfo();?> that Magic Quotes really
really are OFF.
