The Enterprise Architect version of Visio lets you add column definitions to your entities, add foreign key relationships, add indices, add comments at the table and column level, and then generate a build script. It also has a reporting tool for generating a data dictionary. In short, it makes a pretty good stab at being a data modelling tool (imperfectly, but sufficiently for my needs).
