Villie, Might seem a silly question, but do you have gd-2.0.1 installed in /usr/local/gd-2.0.1?
Which version does phpinfo() say you're running? What does the libphp4.so file link to? Type "ldd libphp4.so" in your apache/libexec folder. Perhaps you need to put the path to libgd in /etc/ld.so.conf? I can confirm that RH7 with GD-1.8 RPM and custom built PHP+GD-2.0.1 does work.
